Unraveling the Cellular Complexity of Polyneuropathies with a Multi-Omics Approach

Polyneuropathies (PNPs) are among the most common neurological disorders, leading to progressive motor and sensory impairment. Their exact causes are diverse and often remain unclear. However, identifying the underlying cause and understanding disease mechanisms is crucial—particularly for treatable forms such as immune-mediated neuropathies. To address this, we applied a multi-omics approach combining single-nucleus RNA sequencing, spatial transcriptomics, RNA in situ hybridization, immunofluorescence, and immunohistochemistry to analyze peripheral nerves from a total of 33 PNP patients and 4 controls (365,708 nuclei). Our results revealed unexpected perineurial cell heterogeneity, loss of Schwann cells, and lipid-associated macrophage accumulation in the nerve of patients with PNPs. These changes extend beyond the endoneurium, redefining PNP as a “pan-nerve disease” with focal perineurial hyperplasia and CXCL14 upregulation as potential molecular hallmarks.
